This page is designed to provide an easy way for family members of parishioners, friends of parishioners, or even in cases parishioners themselves who are homebound to request a pastoral visit. Often times when someone becomes ill, the parish office is not always made aware. Because of HIPPA policies it is important for family members, or the individual themselves to notify the parish office if one is hospitalized or enters a nursing/rehab facility. Sometimes we are made aware, but that is NOT always the case.
Each one of us is called to be the hands and feet of Christ, and through our common baptism share in the ministry of Christ. We are all collaborators in ministry and called to work together to bring comfort, healing, and peace to those in their time of need.
NOTE ON THE FORM BELOW: We do request the contact information of the person requesting the pastoral visit because it can be helpful in times when we are having trouble reaching out to the individual and their families.
